How Do I Make an Appointment?

The Client Portal is here for your convenience! Within the client portal, you can fill out intakes, request and cancel appointments, communicate through HIPAA secure messages, pay bills, and more. Plus, it’s mobile friendly! This will definitely cut down on playing phone tag with each other and save you valuable time.

To get to the Client Portal, you can click on “Click Here to Request an Appointment” on this website or clicking the following link: https://chauncey.clientsecure.me/

You can sign up as a New Client after following a few prompts when you enter the Client Portal. After you become a New Client, you’ll be free to request an appointment at a time that works for you. Once you do that, I’ll review your request and either accept or decline the appointment.

When an appointment is accepted, I will then send you an email requesting that you fill out the electronic intake paperwork before our first appointment. The average time for an individual to fill out the forms is about 30 minutes. If you have any difficulty filling out the forms, no worries! Just fill out what you can. We can always go back over the forms and fill them out later together at an appointment.

Once you fill out the forms and submit them, that’s it! You will then be ready to attend our first appointment. If anything is confusing feel free to contact me via email or phone. I look forward to meeting you and assisting you in any way that I can.
